The San Francisco Community Health Center (formerly the Asian and Pacific Islander Wellness Center) is a health services and advocacy organization in San Francisco focused on providing health care and support services to LGBT people, homeless people, people with HIV/AIDS, and people of color in the Tenderloin district of San Francisco. 1

SFCHC is funded in part by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services, by revenues from program services, and by contributions from left-of-center donors such as San Francisco Foundation, Horizons Foundation, and San Francisco AIDS Foundation. 2

Background

The San Francisco Community Health Center (SFCHC) provides health care services and support to “immigrant and communities of color, queer, trans, unhoused people, and all of us who are most impacted by oppression” in San Francisco. 3

SFCHC was founded in 1996 as the Asian and Pacific Islander Wellness Center (API Wellness) with the merger of the Asian AIDS Project and the Living Well Project. The Asian AIDS Project was founded in 1987 by Asian American Recovery Services in response to the AIDS epidemic. The Living Well Project started in 1988 as part of the Gay Asian Pacific Alliance, supporting HIV-positive and gay men. It provided on-site primary care, psychiatric services, and  housing assistance to people living with HIV. 4

In 2006, the HIV programs of the Tenderloin Area Resource Center, which was founded in 1994, moved to API Wellness. 5 In 2012, API Wellness began a Street Medicine program to provide care to homeless people with municipal and federal funding. 6 In 2015, API Wellness became a federally qualified health center through the Health Resources and Services Administration (HRSA) 7 which is part of the federal Department of Health and Human Services. 8

In 2018, API Wellness re-branded as San Francisco Community Health Center (SFCHC) to indicate that, although it would continue with Asian and Pacific Islander-focused programs, it would also provide services to anyone who is in need, especially “all communities of color, LGBTQ communities, and really, the queerest among us.” 9

In 2023, SFCHC opened the first stand-alone facility for transgender and gender non-conforming people in San Francisco, called Trans: Thrive. 10 The program had been a part of SFCHC since 2007 and is sponsored primarily by Gilead Sciences, 11 a biopharmaceutical company that develops medicines such as antiviral drugs used in the treatment of HIV/AIDS and hepatitis. 12

SFCHC is located in the Tenderloin district of San Francisco which is an area known for high levels of homelessness, poverty, substance abuse, and crime. 13

Services

San Francisco Community Health Center provides health care services including primary care, dental, behavioral health, and HIV care to “queer, trans, unhoused, HIV+ neighbors, communities of color, and otherwise historically marginalized people.” 14

In addition to health care services such as hormone therapy and surgery, SFCHC offers services to transgender people through drop-in and community center Trans: Thrive, and the Taimon Booton Navigation Center which provides beds, meals, showers, case management, and community for trans and non-gender conforming people. 15

SFCHC also provides street-based services for homeless people and conducts programs focused on Asian Pacific Islanders (APIs) such as the Joy Luck Club which supports APIs living with HIV. 16

Advocacy

In 2022, San Francisco Community Health Center policy advocacy at the national level focused on funding for AIDS/HIV victims and protection and support for transgender interests. In 2023, it created a Community Policy Board made up of clients and staff to focus on state and city level issues including access to PrEP and to Naloxone and availability of affordable housing. 17 PrEP, or pre-exposure prophylaxis, is a medication used to reduce the probability of getting HIV from sexual activity or drug injections. 18 Naloxone is a medicine used to reverse an opioid overdose. 19

Partners and Funding

San Francisco Community Health Center’s partners include National Minority AIDS Council, National Health Care for the Homeless Council, Coalition on Homelessness San Francisco, HIV/AIDS Provider Network, and CenterLink. 20

SFCHC’s 2022 revenues were $16,014,921 and expenses were $9,987,512. 21Revenues included government grants of $7,432,679, contributions of $6,044,581, and revenues from program services of $1,472,528. 22 It is supported by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services. 23 The health clinic operates on a sliding scale based on a client’s income, so services may be free or at a reduced cost. 24

Donors include San Francisco AIDS Foundation, Asian Pacific Fund, Horizons Foundation, San Francisco Foundation, and donor-advised fund providers American Endowment Foundation, Fidelity Investments Charitable Gift Fund, American Online Giving Foundation, and Charities Aid Foundation of America. 25

SFCHC hosts an annual Show of Hope gala to raise donations. The 2023 gala was its 36th and its honorees included former Speaker of the House Nancy Pelosi (D-CA) and Cecilia Cheung from the Transgender Law Center. 26

Leadership

Lance Toma became CEO of San Francisco Community Health Center in 2006 and was deputy director of the organization from 1999 to 2006.  He was originally from Hawaii. He earned a masters in social work from the University of Chicago. 27 Toma began his career as a clinical social worker before joining SFCHC. He is the board vice president of National Minority AIDS Council; a board member of Communities Advocating Emergency AIDS Relief Coalition; board secretary of CenterLink, 28 a network of LGBT community centers; 29 co-chair of the HIV/AIDS Provider Network; and Commissioner of the California Commission on APIA Affairs. 30

Ben Plumley is the board chair. He is the founder and CEO of Hunuvat Global, a global health access strategy organization that develops policy and advocacy strategies on global health care access; managing director of the Global Listening Project focused on preparedness for future crises; and chairperson of the MTV Staying Alive Foundation working with young people to prevent HIV/AIDS. 31 Plumley “actively supports progressive candidates and programs in the Bay Area and across the USA” including Rep. Barbara Lee (D-CA), former Rep. TJ Cox (D-CA), and voting rights campaign Fair Fight Action. 32
